Sd Card Problem, No Format is Working?

I had tried to format my sd card, but after format when i remount my card to pc the same files appear which i formatted. i have tried all kind of software to format like disk management of windows 7, Sd card Formatter, low level format tool, etc but after format and after re mounting card to pc all those same files are seen. also i cannot copy anything to sdcard cause after copy and re mount those files are lost.
